The Ethnocare Overlay Transtibial is now available in a new Extra Short size, making adjustable prosthetic socket fit accessible to more below-knee amputees.
For individuals with shorter residual limbs, finding a solution that accommodates their anatomy can be particularly challenging.
That's why Ethnocare is introducing the Overlay X-Short, a new version of our adjustable pneumatic prosthetic device, specifically designed for transtibial amputees with very short residual limbs.
Featuring a shorter profile and a redesigned single-row inflatable bladder system, the Overlay X-Short delivers the core benefits of the original Overlay in a more compact design.
Understanding Residual Limb Volume Changes and Prosthetic Socket Fit
A prosthetic socket is designed to provide a secure, comfortable connection between a residual limb and a prosthesis.
However, the volume of a residual limb is not constant.
Throughout the day, changes in fluid distribution, physical activity, and other physiological factors can cause the residual limb to increase or decrease in volume.
These changes can affect how the limb fits inside the socket and may lead to:
-
A feeling of looseness or instability inside the prosthesis.
-
Unwanted movement between the residual limb and the socket, sometimes referred to as pistoning.
-
Increased pressure or discomfort in certain areas.
-
Difficulty maintaining a consistent socket fit throughout the day.
-
Frequent adjustments using prosthetic socks.
For many transtibial amputees, prosthetic socks are a common solution for managing these daily changes.
However, adding or removing socks may require interrupting activities and removing the prosthesis.
An adjustable prosthetic interface offers another way to manage these fluctuations.
What Is the Ethnocare Overlay?
The Ethnocare Overlay is an innovative adjustable prosthetic device designed to help people with lower-limb amputations manage changes in socket fit throughout the day.
Unlike a traditional prosthetic sock, the Overlay uses an integrated pneumatic system to compensate for changes in residual limb volume.
The device is worn over the prosthetic liner and inside the existing socket.
Using a built-in pump and release valve, users can inflate or deflate the air bladders to fine-tune their socket fit as their residual limb changes.
The main advantage? Adjustments can be made without removing the prosthesis.
This technology is designed to support:
Convenient socket adjustability
Add or release air to respond to daily residual limb volume fluctuations without repeatedly removing the prosthesis to change sock thickness.
Improved comfort
Help maintain a more consistent fit between the residual limb and prosthetic socket throughout the day.
Enhanced stability
Help reduce unwanted movement inside the socket associated with changes in residual limb volume.
Greater independence
Give prosthetic users greater control over everyday socket adjustments using a simple, manually operated system.
Designed with a thin textile structure, the Overlay can be integrated into many existing prosthetic setups without requiring a completely new socket.
Introducing the NEW Overlay X-Short
Until now, the Overlay Transtibial was offered in Short and Long lengths.
While these designs accommodate a wide range of residual limb anatomies, some transtibial amputees have residual limbs that are too short to comfortably accommodate the standard Overlay bladder arrangement.
The new Overlay Extra Short was developed to address this limitation.
A Compact Single-Row Inflatable Bladder Design
The main difference between the standard Overlay and the new X-Short version is the arrangement of the inflatable air bladders.
While the original Overlay uses a longer bladder configuration, the X-Short features a single row of inflatable bladders.
This design reduces the overall length of the pneumatic section while preserving the essential functionality of the Overlay.
The result is a more compact device designed for individuals who previously could not accommodate the standard Overlay lengths.
Designed for Very Short Transtibial Residual Limbs
A shorter residual limb can present unique challenges when selecting prosthetic accessories.
The available space between the distal end of the limb and the proximal socket trim line may limit the use of certain products.
The new X-Short design helps address these anatomical constraints by providing a shorter pneumatic profile.
This allows more transtibial amputees to explore the benefits of adjustable socket fit management.
The Same On-Demand Adjustability
Although the bladder arrangement has changed, the underlying principle remains the same.
The Overlay X-Short allows users to:
-
Inflate the pneumatic system to help accommodate additional space inside the socket.
-
Release air when less volume compensation is needed.
-
Make adjustments throughout the day without removing their prosthesis.
-
Continue using a compatible existing liner and socket system.
For prosthetic users dealing with daily residual limb volume changes, this can provide a practical alternative or complement to traditional prosthetic sock management.

Overlay vs. Prosthetic Socks: Managing Volume Changes Differently
Traditional prosthetic socks remain an established method for accommodating residual limb volume changes.
As the residual limb decreases in volume, users may add sock ply to help fill the additional space inside the socket.
When the limb increases in volume, socks may need to be removed.
Although this approach is effective for many people, it can become inconvenient when adjustments are needed frequently.
The Ethnocare Overlay approaches the same challenge differently.
Rather than relying exclusively on additional textile layers, its integrated pneumatic system allows users to adjust the air volume inside the socket.
This offers several practical differences:
|
Traditional Prosthetic Socks |
Ethnocare Overlay |
|---|---|
|
Uniform volume compensation: Adds thickness around the residual limb, including areas that may not need additional pressure. |
Targeted volume compensation: Inflatable bladders help fill specific areas where additional support is needed. |
|
Potential alignment changes: Adding sock layers can alter how the residual limb sits within the socket. |
Helps maintain alignment: Adjusts volume within targeted areas to help preserve the intended socket fit and limb positioning. |
|
Requires removing the prosthesis: Adding or removing socks interrupts daily activities. |
On-demand adjustability: Inflate or deflate the Overlay without removing the prosthesis. |
|
Limited cushioning: Traditional prosthetic socks primarily compensate for volume through textile thickness. |
Additional cushioning: Inflatable air bladders provide adjustable cushioning between the liner and socket. |
|
Fixed adjustment increments: Volume compensation depends on the thickness and number of sock layers. |
Progressive adjustments: Air volume can be increased or decreased gradually to fine-tune the fit. |
|
Suspension challenges: Volume loss may contribute to unwanted movement and reduced suspension effectiveness. |
Improved suspension support: A more secure socket fit may help improve suspension effectiveness with compatible systems. |
|
Potential pistoning: A loose socket may allow excessive vertical movement during walking. |
Reduced pistoning: Improved socket contact and stability may help limit unwanted vertical movement. |
|
Pressure distribution: Additional sock thickness may increase pressure in areas that are already adequately fitted. |
Localized support: Targeted inflation can help improve contact in areas affected by volume loss. |
Both approaches have their place in prosthetic care, and the most suitable option depends on the individual's needs and clinical circumstances.
The Overlay is intended to help manage daily volume fluctuations within an appropriately fitted prosthetic socket. It is not a replacement for necessary socket modifications or a new socket when these are clinically indicated.
What Does Clinical Experience Tell Us About the Overlay?
Beyond the product's mechanical design, the ability to manage prosthetic socket fit throughout the day has practical implications for daily life.
In an Australian clinical case study of the Ethnocare Overlay, eight participants evaluated their experience with the pneumatic volume management system.
Among the reported findings, the average number of times participants removed their prosthesis daily to manage socket fit decreased from 2.2 to 0.6.
Participants also reported improvements in measures of comfort, stability, ease of adjustment, and volume management.
These results are encouraging, although the small sample size means they should not be interpreted as definitive evidence of effectiveness across all prosthetic users.
Nevertheless, the findings illustrate why convenient socket adjustability can be meaningful for people who experience frequent residual limb volume fluctuations.
